This website is managed by the Transition Support Project and aims to share resources around mental health and the NDIS.

The Transition Support Project is based at Flinders University, and is funded by the Department of Health, Disability and Ageing. The project primarily helps Commonwealth-funded community mental health services transition their clients to new NDIS supports.

This website makes publicly available many of the resources we have developed.
